چکیده

To date, there has not been direct visualization of the anatomic location of direct His bundle pacing (DHBP) leads in the human heart. The absence of such data has contributed to disagreement about the location of DHBP leads with respect to the plane of the tricuspid valve.1,2 We present an autopsy study of a patient who had previously had a DHBP lead implanted, showing unequivocally that the lead is implanted on the atrial side of the tricuspid annulus. An 81-year-old man with diabetes died of sepsis secondary to a lower-extremity infection. Two years prior, the patient presented with symptoms of congestive heart failure and presyncope. He had a history of coronary artery bypass graft and myocardial infarction with mild to moderate left ventricular dysfunction (ejection fraction, 40%– 45%). -blockade therapy was limited by sinus bradycardia and frequent Wenckebach block. The patient was noninducible for ventricular arrhythmias, and a pacemaker was recommended for chronotropic incompetence and AV block. To prevent pacemaker-induced electric dyssynchrony, we implanted a DHBP lead. DHBP lead implantation was performed as previously described.3 Briefly, an octapolar mapping catheter was

Overcoming left bundle branch block by permanent His bundle pacing: Evidence of longitudinal dissociation in the His via recordings from a permanent pacing lead

Introduction Narrowing of the QRS and overcoming left bundle branch block (LBBB) with His bundle pacing (HBP) has been previously described. We present the first 2 reported cases of left bundle branch (LBB) delay with evidence of a split His electrogram during unipolar mapping from the tip of the His bundle (HB) lead during pacemaker implantation. These findings suggest that the site of LBB del...

Step-wise Approach to Permanent His Bundle Pacing

There has been a recent upsurge of interest in permanent His bundle pacing, given increased understanding and appreciation of the role dyssynchronous ventricular activation plays in congestive heart failure and atrial fibrillation. Permanent His bundle pacing gives implanters the ability to avoid causing ventricular dyssynchrony in patients dependent on ventricular pacing, and can provide an al...

Case Report Safe Right Bundle Branch Block Pattern During Permanent Right Ventricular Pacing

It is known that an electrocardiogram (ECG) after transvenous right ventricular (RV) pacing should yield left bundle branch block (LBBB) QRS patterns. When right bundle branch block (RBBB) pacing morphology appears in a patient with a permanent or temporary transvenous RV pacemaker, myocardial perforation or malposition of the pacing lead must be ruled out, even though the patient may be asympt...

Acute His Bundle Injury during Permanent His Bundle Pacing: Mechanistic Insights into Intra- Hisian Block

Permanent His bundle pacing (HBP) is a physiological alternative to right ventricular pacing. Acute injury to the His bundle can occur during permanent HBP in the form of transient or permanent bundle branch blocks. Observations of mechanical injury to the His bundle during HBP lead fixation and electrophysiological response to HBP provides additional mechanistic insights into the pathology of ...

Hyper-response to cardiac resynchronization with permanent His bundle pacing: Is parahisian pacing sufficient?

The feasibility of resynchronizing ventricular activation by permanent pacing of the His bundle region has been previously described, and has clinical advantages over traditional right ventricular (RV) apical pacing.1–4 The physiologic benefit of permanent His bundle pacing (HBP) is the ability to stimulate the ventricles through the intrinsic His-Purkinje system, which results in synchronous e...
